During maintenance patrols, the refilling of the FM200 gas cylinder was scheduled. After refilling, it was observed that the pressure was not stable, which caused the gas cylinder in the extinguisher to discharge, resulting in damage.

Cause of the Incident

Failure to follow safety and security procedures and instructions, including the contractor’s haste and negligence, which is unacceptable to the Supreme Authority for Industrial Security.
